This form can be found on the Department’s website “forms” link or can be obtained from the Department’s offices at 111 Maiden Ln.

Because offenses may be reclassified upon further investigation or if new information comes about, the numbers contained in these reports are not absolute. For annual statistics, please see the Department’s most recent Annual Report, available under the "publications" link. The State Law Enforcement Division (SLED) provides official crime statistics, which can be found on its website. The offense categories and descriptions are based on guidelines from the FBI and SLED.
